Aven, Inc. ("Aven," "we," "us," or "our") is a Delaware corporation providing a caregiver performance engineering platform for community-based care organizations, including PACE programs, in-home hospice agencies, and Medicaid Home and Community-Based Services (HCBS) providers. This Privacy Policy explains how we collect, use, and protect information when you visit aven.care (the "Site") or submit an inquiry through our forms.

When you submit an inquiry through our Advisory Council or Pilot Partnership forms, we collect the information you provide, which may include your name, organization name, role or title, email address, phone number, care setting and organization size, state or region of operation, and any additional information you choose to share.

Aven does not collect, request, or process protected health information (PHI), participant or patient records, or any clinical data through this Site. Our forms are limited to business-contact and organizational information for the purpose of exploring an advisory or pilot partnership relationship.

Like most websites, the Site may automatically collect limited technical information when you visit, such as browser type, device type, general geographic location, and pages visited.

How We Use Your Information

We use the information you provide to respond to your inquiry, evaluate whether your organization fits our current program focus, communicate with you regarding your inquiry, and maintain internal records of prospective partners and advisors. We do not use submitted information for advertising, do not sell it to third parties, and do not use it to build advertising profiles.

Text Message (SMS) Consent

If you provide your phone number and affirmatively opt in, Aven may send you non-marketing text messages related to your inquiry — for example, to confirm receipt or schedule a follow-up conversation. Message frequency varies. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for assistance. Consent to receive text messages is never a condition of any partnership, pilot, or advisory relationship with Aven.

How We Share Information

We do not sell your information. We may share information with service providers who help operate this Site and process form submissions — currently AlphaBlossom, our form-processing platform — under obligations to protect your information; with Aven personnel and advisors directly evaluating your inquiry; or as required by law.
